Whilst in international waters, there are no laws against the consumption of cannabis. The United Nations Convention on the Law of the Sea does not specifically prohibit cannabis use, nor that of other medications/drugs. However when entering territorial waters of any particular nation, you will immediately be subject to local law as outlined above.
There are no known differences in restrictions to travelling with medication when entering a country by sea, as compared to air or land travel.
